Performance reviews, salary increases, and bonuses discussions are not meant to be surprise and delight (or fright) moments. 

They are a memorialization and financial representation of ongoing conversations.

If you’ve maintained those conversations, as you administer the process, you can begin the dialogue with “as we have discussed.”  If you have not, own it and consider how you will guide a context and content-rich conversation about this past year.
